In order for us to change your name in our database, we will require legal documentation that indicates your legal name, such as a passport, birth certificate or state identification. If your name has changed due to recently becoming married, a copy of your wedding certificate will suffice. Please fax a copy of this documentation to 661-793-6555 and address the fax to the attention of "MCP Escalations." On the fax cover sheet, please include your name as it is currently listed in our database, your MCP ID and the way you would like your name listed in our database. As an alternative to faxing, you may scan the documentation and reply to this email, including the scanned image of the document as an attachment. Please note it is recommended that you contact the testing center to ensure that your name is correct in their database as well. When contacting your testing center, please have a copy of your record available.
