Translating some English questions to Arabic

Dear professors
Can you please help me in translating some English questions from my questionnaire to Arabic. Actually, I find a difficulty to do that, not because I do not understand the questions, but because it is difficult to find suitable Arabic words. I am doing a research and I have built my research tools, and one of them is questionnaire. Actually, I need to translate the English version to Arabic and I have finished from it, but some questions are unclear, so I am writing to you to help me with the following sentences
